What this test measures

Much of it travels in the blood attached to carrier proteins, and a small amount circulates unbound. The Total T4 test measures both the bound and unbound forms together.
Because carrier protein levels can change, total T4 results can be affected by factors such as pregnancy, liver disease, and some medications. For that reason, total T4 is often reviewed with free T4, thyroid-stimulating hormone, and sometimes T3. A blood draw is the only sample needed, and fasting is usually not required.

It can also be used to monitor a known thyroid condition.
