What this test measures
The thyroid gland produces hormones that influence metabolism, heart rate, body temperature, and energy use. Thyroxine, also called T4, is the main hormone the gland releases. Much of it travels in the blood attached to carrier proteins, and a small amount circulates unbound. The Total T4 test measures both the bound and unbound forms together.
Because carrier protein levels can change, total T4 results can be affected by factors such as pregnancy, liver disease, and some medications. For that reason, total T4 is often reviewed with free T4, thyroid-stimulating hormone, and sometimes T3. A blood draw is the only sample needed, and fasting is usually not required.
The test may be ordered when a person reports symptoms such as unexplained weight change, fatigue, feeling cold or overheated, or a racing heartbeat. It can also be used to monitor a known thyroid condition. A single total T4 result does not diagnose a thyroid disorder on its own; clinicians interpret it in the context of other hormone levels and the clinical picture.
